Eligibility Criteria

Inclusion Criteria
* Clinical diagnosis of NVAF
* CHA2DS2-VASc score greater than or equal to 2
* New patient visit for initiation of oral anticoagulation at primary care and cardiology clinics

* Unable to provide informed consent
