* Resident of Harris County, Houston Texas
* Community-dwelling
* Substantiated Self-Neglect
Exclusion Criteria
* Non-community dweller
* No available for projected follow-up period
* Impaired decision-making with no available proxy
* Already receiving care from TEAM or within the past year
* Terminal Illness
