Eligibility Criteria

Inclusion Criteria
* the patient to the community medical institutions;
* ASSIST score is in 12-27, in a moderate risk;
* voluntary to participate;
* to agree to complete baseline interviews and follow-up studies

* serious physical illness, action inconvenience.
