* Grafted vessel
* Left ventricular ejection fraction \< 30%
* Severe calcification and/or severe tortuosity
* Uncontrolled coronary spasm
* Life expectancy \< 2 years
* Planned high risk surgery
* Pregnancy or breast-feeding
