* AHI \> 30
* Intolerance to CPAP therapy
* Signed informed consent

Exclusion Criteria

* Epworth sleepiness scale \> 16/24
* Severe cardiac and/or respiratory disease
* BMI\>32 kg/m2
* Dental contraindication to oral appliance
