\- Adult patients ventilated more than 48 h

* Stable respiratory and hemodynamic conditions for SBT
* Consent of patients
* Arterial line

Exclusion Criteria

* Laryngeal dyspnea
* Tracheostomy
* Arythmya
* No echogenicity
* Paraplegia \>T8
