Title/Comparison of Pilates Exercises vs. Aerobic Exercises in Primary Dysmenorrhea . Background Primary dysmenorrhea (PD) is a common gynecological condition affecting young women, characterized by painful menstrual cramps. Non-pharmacological interventions like exercise have been suggested to reduce pain and improve quality of life. Pilates and aerobic exercises are popular interventions, but comparative evidence is limited. This study aims to evaluate and compare their effectiveness.Purpose of the study:

This study will conducted to examine the superiority of effectiveness of Pilates exercise and aerobic exercise on pain and quality of life in female from primary dysmenorrhea. Group A: Patients will receive Pilates exercise, the exercise program was repeated 3 times 8 weeks 30 minute for each leg in session.

Group B: Patients will receive aerobic exercise, the exercise program was repeated 3 times 8 weeks 40minute for each session.

Inclusion Criteria:

1. Females with Primary dysmenorrhea
2. Age between 18 to 25 years
3. Having regular menstrual cycle of 28 - 35 days
4. Individual with BMI 18.5 to 24.9 kg/m2

Exclusion Criteria:

1. Acute or chronic pelvic pathology
2. Subjects having any history of regular exercises 3days/week \[daily average 30-45 min\]
3. Attending physical activity like swimming, running or fitness center
4. Who cannot perform exercise
5. Not willing to participate

Assessment Inclusion Criteria:

1. Females with Primary dysmenorrhea
2. Age between 18 to 25 years
3. Having regular menstrual cycle of 28 - 35 days
4. Individual with BMI 18.5 to 24.9 kg/m2

Assessment Visual analogue scale Used to identify the menstrual pain severity. Scores are based on self-reported pain measurements Short form health survey (SF-12) It was used for assessment of quality of life for both groups (A \& B) before and after treatment.

Verbal Multidimensional Scoring system The VMS grading system, which assigns scores from 0 to 3, was categorized into mild, moderate, and severe based on the extent of activity limitations and pain experienced Data collection and Statistical analysis Data collection

1\. Data of all patients in both groups will be collected and include: Name, serial number, age (years), weight (kg), height (cm), BMI (kg/m2). Primary dysmenorrhea will measure severity of pain by visual analogue Scale before and after each 8-week period.

b. Statistical analysis

For testing the study hypotheses, the following statistical methods will be used:

1. Descriptive statistics (mean and standard deviation) of demographic data of all patients in both groups.
2. Suitable statistical tests will be used according to the nature of the collected data
3. Level of significance for all tests will be set at p-value ≤ 0.05
4. The statistical package for social sciences (SPSS) version 26 for windows (Armonk, NY: IBM Corp) will be used for data statistical analysis

Participants will be randomly assigned to one of two parallel groups: Pilates exercise group or Aerobic exercise group. Each group will follow a supervised 8week exercise program."
