* Age \> 18 yr.
* Hospitalized to Siriraj Hospital
* Infections with ESBL-producing E.coli or K.pneumoniae
* Consent informed consent

Exclusion Criteria

* Pregnancy or lactating mother
* Colistin allergy
* Neurologic diseases
