* significant cardiovascular disease (angina, myocardial infarction, cerebrovascular disease, or significant arrhythmia in the preceding 6 months)
* uncontrolled diabetes mellitus (serum glucose level ≥140 mg/dl)
* insulin dependent diabetes mellitus (type I)
* secondary hypertension
