Eligibility Criteria

* 45-75 years
* Body Mass Index \>= 25
* Diabetes treatment for at least 12 months
* Regular visitors of the participating community pharmacy

Exclusion Criteria

* Patients taking only insulin (no oral antidiabetic drugs)
